Ibis provides expert facilitators to help you conduct hazard studies and risk assessments of your projects and activities to identify the issues, assess the risks, establish the robustness of your existing controls and decide on residual risk. Our team based approaches use leading edge methods that are proven and accepted by industry and regulators.

We use a range of methods consistent with relevant Australian Standards to ensure that the resources of the team are optimally utilised. We have considerable experience in many methods including:
  • HAZOP;

  • What if?;

  • Layer of Protection Analysis;

  • Failure Modes and Effects Analysis;

  • Team Based Risk Assessment Methods.

Our methods can be applied throughout the life cycle of a project from concept design, detail design, operations and decommissioning.

We develop risk analysis tools to help you quantify risk in terms that your people understand. We select and calibrate the risk analysis tool to each client so that it is effective in distinguishing those risks which require attention.

Our Approach
Our approach to hazard studies and risk assessments is based on considerable experience of how to ensure that risks are effectively identified and analysed using the optimum resources for the purpose. Key steps in our approach include:

  • Scoping and preparation. We work with you to define the scope and to prepare including advising on the method, the risk assessment tool and the size and content of the team.

  • Facilitation. We facilitate the hazard studies and risk assessments to optimise the use of the team and to ensure that risks are adequately assessed.

  • Documentation. We ensure that each hazard study and risk assessment is well documented so that it is useful long after the event. Our documentation assists management to demonstrate due diligence. Many clients use the information to improve training and to improve their risk management systems.

  • Training for your facilitators. We can also help you become self sufficient by training your people to facilitate hazard studies and risk assessments.

Our Experience
Our consultants have successfully completed a variety of hazard study and risk assessment assignments for major oil and gas, petrochemical, manufacturing, mining and infrastructure organisations.
Our work includes:
  • Ibis has conducted a large number of risk assessments of Origin Energy's LPG terminals in Australia and in the Pacific region. These risk assessments have been conducted to assess the controls, systems and management processes for each of the terminals.

  • We have conducted numerous site risk assessments for Origin Energy to help establish risk profiles and evaluate the current control systems.

  • Ibis was commissioned by Iluka Resources operations staff to conduct a combination of HAZOP and What if? investigations into the operations and maintenance activities associated with the synthetic rutile plant. These investigations followed a number of incidents and identified areas where process safety management systems required upgrading.

  • Ibis has conducted risk assessments for a range of projects for the chemical manufacturer Penrice Soda Products. These include filter replacement and, plant upgrades. In addition, we conducted plant wide risk assessments for the process plant and a whole of mine risk assessment for the limestone mine. This has been pivotal in supporting the client's insurance renewal processes and also in generating an effective risk register for the operations.

  • Ibis has conducted Layer of Protection Analysis for high consequence risks at the Leinster Nickel Operation of BHP Billiton. These analyses enabled the operators to evaluate the risk control systems and their reliability and identify areas where the control systems needed upgrade.

  • Ibis has conducted Layer of Protection Analysis of catastrophic risks associated with the proposed Perseverance Deeps Project for BHP Billiton. The analyses were used to assist in improvement of the design to ensure that appropriate layers of protection were implemented to ensure safe operations.

  • Ibis has also facilitated risk assessments of the Kestrel Mine Expansion Project as well as conducting Occupational Health Risk Assessments for Rio Tinto Coal.
